Informed Consent for Telepsychiatry

By booking and attending a session with Isabel, you voluntarily acknowledge and agree that:

• Remote medical care involves real-time video and audio transmission.
• You must be physically located within one of the authorized states — Florida, Oregon, Washington, South Carolina or Delaware — during the exact time of each consultation.
• In the event of a technical disruption during the call, your provider will immediately reach out to your registered phone number.
• You explicitly consent to be contacted regarding your health status and care reminders via phone, email, or secure messaging.
