We call the patient
Within 2 to 3 business days we contact the patient and schedule the no-cost 15-minute initial consultation, in Spanish or English. If you would rather coordinate the appointment yourself, say so in the form.

We coordinate the next steps
Isabel Zapata, APRN, PMHNP-BC, provides the psychiatric evaluation and, when appropriate, medication management. You maintain continuity of primary care. With the patient's authorization, we share the evaluation and care plan.
COORDINATED CARE
When a specialized mental health evaluation may be helpful.
Primary care is often the first setting where anxiety, depression and other mental health needs are identified and treated. In some cases, complementing that care with psychiatric evaluation, diagnosis or specialized medication management can help define the next step.
Situations in which a referral is often considered
Insufficient response to treatment
Anxiety or depression symptoms that persist despite one or more medication trials at an adequate dose and duration.
Diagnosis to be clarified
Diagnostic questions that call for a more complete evaluation; for example, when adult ADHD or bipolar disorder is suspected.
More complex medication
Several psychiatric medications at once, side effects that are hard to manage, or a need for fine adjustments and close follow-up.

From Oasis Mental Care
Isabel Zapata, APRN, PMHNP-BC, provides psychiatric evaluation, diagnosis and medication management for adults ages 21 to 64, by video visit, in Spanish or English.
Coordination between primary care and psychiatry helps keep the patient from being split between isolated services and lets both clinicians consider the full picture, including drug interactions.

Before sending the referral
Does Isabel replace the primary care provider?
No. You remain in charge of the patient's overall health. Isabel Zapata, APRN, PMHNP-BC, provides psychiatric evaluation, diagnosis and medication management, and shares the plan with you with the patient's authorization.
In which states can the patient be seen?
Florida, Oregon, Washington, South Carolina, and Delaware. The patient must reside in one of those five states.
Will I receive information about the plan?
Yes, with the patient's authorization. We share the evaluation and medication plan with you, so you can account for possible interactions and the full health picture.
What happens after the form is sent?
We review the referral and, based on the selected preference, contact the patient or coordinate with the professional within 2 to 3 business days.
